HELP! LOST MY ALEVELS CERTIFICATE

yes i m irresponsible and take full responsibility of it but gratefully i m in university now so i dont need it for university admission. but i still need it for some award ceremony. i gave my alevels general paper exams in nov 2011 and got results in jan 2012. and my certificate just vanished after it.

can anyone please tell me how to get reissued? and how much does it cost?
