Contact the British Council in your area.They will give you the form. fess structure,the option code and list of subjects.
Just fill in that form,attach necessary documents and pictures and submit that.
They will guide you. They'll tell you when to collect your statement of Entry.
Just decide which subjects you want to give and which session.
Just one thing I wanted to ask.Are you giving AL or OL?
