About forecast grades for University

Anyone can give me a format of the paper to print and give to each of my teacher for them to put the grades in it?
I went to school and they told me they do not give the forecast they sent to cambridge, and that I should go get my own by myself..

Pls urgent today asap I need it( maybe tommorow I can run and find a printer to print it)

I mean what are you guys using to apply for university?
