Need help with the form...

See guys in the form it says... Retake and first time... M giving Physics again..
What should i write?? plus deadline for first time is 16 August and Retake is 9 September...
and retake it requires Result of may/june... what should i do??? M really confused... Thanks...

When applying for a retake, you must attach a copy of your statement of results and your previous center and candidate numbers.
The extra time for retake candidates is to get their hands on their 'Statement of results'.
Once you get that, then submit the form.
