As far as I remember, an account or statement has no particular format. You just need to plan your paragraphs and decide on the sequence of events before penning it down.

Is there a heading that we should put...or just write continuous

Write down a heading if the question asks for it. Otherwise, I think continuous prose is all that's needed but you can be on the safe side by mentioning a heading anyway. Do verify this from your teacher.
