this is a easy question and I don't think you have any problem with setting the query or identifying that the "If...then...Else" rule should be used ... I'm assuming you are having problem with setting the "If...then...Else" rule. Instead of putting yes or no in the Compare to box use... -

I'm not sure about the index field thing but in order to create relationship, you'll actually need to create a source file yourself using the word document provided. It'll help you if you create a table like this in excel and then export it into access. And yes I did manually the relationship... -
