erm i think they do matter :S
my teacher said so n thats the reason everyone is going crazy to get authentic ones :p
it matters only because its for the sake of leaving a good impression on the examiner .. there are no marks for it .
so technically , it doesnt matter :p

primary source : CIE

I'm not sure if there are marks regarding the format but it is important.
In one of the examiner reports, the checklist provided had a point

~ Appropriate/ Correct format used.

So perhaps there may be a mark allocation for that. Don't know.
However, just use the correct format. You don't wanna take a risk lol :p

i m very unsure abt these formats cz i never saw a directed task saying dat begin wid the address of the reciever/sender. mostly they say start wid Dear--------
so wat to do in this situation where they themselves direct how to start

Well, if you have ever seen an official letter from a company or some organization then the sender's address is always there.
The question won't tell you to start off with an adress. It is understood that it is a correct format and it should be followed :)
