I couldn't find any relevant materials in the printed math and physics textbooks I have. Do I need to print these materials specially? Where can I find them? Or will they be provided to me along with the exam paper in the examination hall?

As a non-native English speaker, should I fill in my surname in all uppercase English spelling? For example, if my surname is Liu, should I write LIU? Also, does "other names" refer to my first name?
