Is Mbbs in pakistan and further specialization somwhere Abroad (USA) a good and possible option??

hi people. i'm about to complete my A levels and i have yet to decide between engineering and medicine... medicine interests me though i find maths simpler...
coming back to the topic, i need some senior (or general) guidance to what i have in mind, i.e do mbbs in pakistan and go abroad for further specialization Like in Cardiology....
i am well aware of the fact that in Pakistan there's alot of competition in the respective field...
so please if anyone has anything to say to help me out, feel free. My Career Could depend on what you say and make me think...
