Which country do you belong to?
Psychology is a studious subject. You have to work constantly hard to achieve success in it. As far as medicine is concerned, Chemistry, Biology and Physics all are compulsory for admission into the medical colleges of Pakistan. In other countries, sometimes other subjects in lieu of Physics may be kept.

I'll advise you to stick to the subjects that you already have, because of your interest in medicine. I thought of the same, to take psychology in A2, but clearing up 4 subjects is hectic enough..you wouldn't wanna opt for a fifth one =/ It's not that difficult to get a good grade in Psychology though, but still..try covering it up in the summers, if you can't, then don't take it.

Then you can't even think of dropping Physics, given that you are interested in opting for medicine in the future.

Stick with your current subjects. Moreover, you don't even need English to pursue a medical career but if it intrigues you, it's good enough for an extra subject.
