Help needed with appearing as private candidate for o level october 2012 session

Salam everyone
I want to appear in the oct/nov 2012 session, for additional mathematics, as a private candidate and i have no idea how to register
Can somone please link me to the form for november 2012, because i can only find the form for may 2012, and i think I'm already late in registering.

On the registration form there is an option if we want to choose CNIC or Passport as our identification document...ive chosen passport....
Can u plz tell me that if v choose passport then what r v supposed to write in the column of "ID number" and it is written mandatory next to it...please help ASAP!!
