As for me,
Do wudu and read the Kalimah. (I heard that a person in Wudu and who dies with the Kalimah on his lips goes to Jannah ) Face the Qiblah and pray for Allah to forgive me, protect me on the Day of Judgement, give me a chance to be near Prophet Muhammed (SAWS) and Hazrat Fathimah and to make my transition easy and give my dear ones Sabr.

That's a tough question, I think.
I would ask people to forgive me, and yes, will try to make the best Salah; read Quran; say Istighfar, and ask Allah S.W.T. to forgive me.
