Urgent help!

What if they ask you to start a letter "Dear principal"

Will we include the format of a letter? or will we just write Dear Principal, and begin?

Secondly, from what I've always studied, a general report and an account have the same format:
To
From
Date
Subject
---------
--------
--------
Signature

Is that true?
And does someone have the format for a leaflet. Answers appreciated. Thanks!
